                                                                 INSTRUCTIONS FOR SUBMISSIONS

To qualify for this program, the individual being honored must have served in one of the United States Armed Forces. First priority will be given to Veterans from each location ( Born, Raised, or Resided), then veterans from outside the Greater Mohawk Valley area will be considered. This form can be submitted online, or may be downloaded, printed and sent in, following the directions on the downloaded form. Pictures and payment also need to be submitted. A 'proof' of the banner will be sent prior to printing. The proof MUST be approved within a week of receipt, or banner will not be printed.

All donations will be used to purchase hardware to hang the banners.
